Drug Class and Mechanism:

ISOWEL Capsule Isotretinoin is an oral drug classified as a powerful treatment for severe acne. Acne, which often affects both teenagers and adults, results from skin inflammation caused in part by the excessive secretion of sebum, an oily substance produced by the skin’s sebaceous glands. This sebum provokes inflammation and eventually leads to scarring (keratinization). Isotretinoin’s precise mechanism of action is not fully understood, but it is believed to reduce acne by decreasing sebum secretion, resulting in less inflammation and keratinization

ISOWEL Capsule should be stored at room temperature between 1530°C (5986°F) and protected from light

Prescribed For:

Isotretinoin is prescribed for the treatment of severe acne that does not respond adequately to other conservative treatments such as creams, drying agents, and topical or oral antibiotics. Significant improvement is often observed in many patients after a 1520 week course of isotretinoin. However, due to the potential for serious side effects, its use should be carefully monitored

Dosing:

The recommended dosage of isotretinoin typically ranges from 0.5 to 2 mg per kilogram of body weight daily. This daily dose is usually divided into two doses and administered over 1520 weeks. Isotretinoin should be taken with food to enhance absorption

Pregnancy:

Isotretinoin is closely related to vitamin A. Using both vitamin A and isotretinoin simultaneously may result in vitamin A-related side effects. Simultaneous use of tetracycline (Achromycin) and isotretinoin should be avoided, as it has been associated with brain swelling (see side effects below). Isotretinoin should not be used during pregnancy. Women of childbearing age must have two negative pregnancy test results before starting therapy and must undergo monthly pregnancy testing during treatment. Two effective forms of birth control should be used during therapy, and pregnancy should be avoided for at least one month before, during, and after stopping isotretinoin

Nursing Mothers:

ISOWEL Capsule It is unclear whether isotretinoin is secreted in breast milk. Nevertheless, due to its potentially serious side effects, nursing mothers are advised not to use it

Side Effects:

Common side effects of isotretinoin include dry skin, dry nose, nosebleeds, chapped lips, and eye redness. Joint pain is also frequent. Patients may experience increased blood cholesterol and triglyceride levels. Psychiatric problems such as depression, hallucinations, and suicidal behavior have been reported. Rare side effects include skin infections, sun sensitivity, hearing impairment, and hepatitis. Isotretinoin can also rarely cause brain swelling (pseudotumor cerebri or intracranial hypertension), leading to symptoms like nausea, vomiting, headache, and vision changes. Severe skin reactions such as Stevens-Johnson syndrome and toxic epidermal necrolysis have been associated with isotretinoin and may be life threatening. Regular monitoring for severe skin reactions is essential, and isotretinoin should be discontinued if necessary
